Dr. Cook makes every effort to be
available for your convenience.
To schedule an appointment with us,

located at 7910 Ustick Road, Boise, Idaho 83704.

- Dr. Cook works by
appointment which provides efficient scheduling.
- Someone will always be
available to schedule an appointment for you during our regular
office hours.
- For your convenience, we
try to do surgery at the same appointment as consultation, however,
sometimes consultation is preferred on a date prior to surgery.
- When making an
appointment please let us know the telephone number where you can be
reached during the day, as well as your home telephone number.
- If you are unable to keep
your appointment or if you are going to be late please call the
office as soon as possible. This courtesy allows us to be timely and
efficient in our office service to other patients.
Please assist us at the time of your
initial visit to the office by providing the following information:
- Your referral slip and x-rays from
your referring dentist. If your referring dentist has taken recent
x-rays, you may request that they be forwarded to our office. If
additional films are necessary, they will be taken at our office.
- Because certain procedures are
considered by insurance companies to be medical in nature, if you
have coverage from a medical plan, please bring
any necessary referral slips from your primary care physician. In
the unlikely event Dr. Cook is not available, he will have made
arrangements with another oral and maxillofacial surgeon to cover
emergencies.
Should an emergency arise, call the
office immediately. Advise the receptionist of the situation.
In most cases, our professional office
staff can help you with your questions. If the doctor is needed, he will
return your call promptly.
Surgical
Fee and Payment:
- Our fees are determined
by the complexity of the procedure.
- We encourage immediate
payment for services at the time services are rendered.
- To assist you in your
payment, we offer the availability of all major charge cards.
- You are responsible for
fees charged for treatment regardless of your insurance coverage.
- We request that each
patient bring complete insurance information at the time of
surgery and our staff will submit the insurance claim on your
behalf. We will assist you in obtaining the maximum benefits
available to you under the terms of your policy.
- Together, we will make a
reasonable and appropriate financial arrangement. May we support
each other in honoring those arrangements.
- If any questions or
problems arise regarding your account please call our receptionist.
Many times a simple telephone call will avert misunderstandings.
Filling
Out Forms
- On your first visit the
receptionist will ask you to fill out a Patient Information and
Health History Questionnaire.
- This questionnaire is a
screening device identifying potential health concerns.
- Please fill out all forms
accurately and completely (health, personal, billing, etc.)
Length Of
Appointment
We understand the importance of your
time and we will make every effort to be prompt, however, due to the
nature of our practice and the use of general anesthesia, it is at times
very difficult to stay on schedule.
Please be assured that Dr. Cook will
provide the same careful attention during your treatment.
If delay occurs, we appreciate your
patience.
